Our places are home to thousands of people. In the North they take care for around 575,000 people and in the South around 375,000 people. They do amazing work understanding the wider health and wellbeing needs of their communities and putting in place the support local people need.
Sometimes though, this work needs to take place even closer to home. Which is why we have set up Integrated Neighbourhoods. These local teams typically are for between 30,000 to 50,000 people and are based around groups of GP Practices. They are the evolution of our 21 Primary Care Networks, and they will build partnership to bring together all health and care services within their local communities.
